By signing below, I, the undersigned parent or legal guardian of the minor participant, hereby give my consent for their participation in all RA Tennis Academy and Club events and activities, including the RA Open. I acknowledge that my child will adhere to all rules and guidelines established by the organization and understand the inherent risks associated with tennis activities.
I hereby release and hold harmless RA Tennis Academy and Club, its staff, volunteers, and affiliates from any and all liability, claims, or demands arising from any injury, illness, or damages incurred during participation in any academy or club activity, including the RA Open. I acknowledge that participation involves inherent risks and accept full responsibility for any personal injury, property damage, or loss sustained by myself (or my child) while on the premises or engaged in activities.
I grant RA Tennis Academy and Club at the RA Open the right to capture, use, and publish photographs and video footage of my child for promotional, educational, and informational purposes across all platforms. I understand these images may be used in print, online, or other media to promote the academy and its events.
In the event of illness or injury, I authorize RA Tennis Academy and Club staff at the RA Open to administer basic first aid to myself (or my child) and to seek further medical treatment if necessary. I understand that reasonable efforts will be made to contact me (or the emergency contact provided) in the event of a medical emergency.
